• Name of Entity filing report
• Registered Agent name, email, and address
• Principal Office address
• Name, title, post office address of each manager (only if LLC is manager-managed)
• Contact Information (email required) for communications
• Payment can be made with credit card or E-Check/ACH
• Filing Fee $25.00 per report year.
• Portal Fee $1.00
